We provide person centred services based on consultation with the service user, those acting on their behalf and the Local Authority assessment of individual needs. This is achieved by listening to people who use the service and / or those acting on their behalf and the staff who support them. Through informed assessment and effective support planning, assessments are made balancing choice, decisions, risks and needs.
